To access the “Online Student Medical Form” login into your FACTS Family Portal. On the left side of the screen, under the subtitle “School Home,” find and select “Web Forms”, then select “Student Medical Form”. Each child that is enrolled at CHRISTIAN will be listed. Click on the bar next to their name to complete the form. Repeat for each child listed.
VERY IMPORTANT: Type your name and date completed at the bottom of the form and hit the SAVE button. When this form is completed on a mobile device, it often does not save the information correctly.
