Submit one form per participant. A non-refundable registration fee per student is required at time of registration. Registration fee includes CSD Camp t-shirt. Registration fees: $30 for current CSD students; $40 for non-CSD students.
OPERATING HOURS/LATE FEES
CSD Summer Camp will run from 7:30 a.m. to 3:30 p.m. Monday through Friday. Aftercare is available from 3:30 to 6:00 at the cost of $7 per day per child. Children may not be dropped off before 7:30 a.m. Children must be picked up by 6:00 p.m. Late fees will be assessed, as follows, for any late pickups: $5 for the first fifteen (15) minutes beginning at 6:01 p.m., $1 per minute beginning at 6:16 p.m.
All camp fees must be paid in full by the first day of camp attendance each week. Fees will be charged based upon the dates indicated below. Make checks payable to Christian School District, or through the link sent to your email.
CAMP FEES (Includes all activities; Students to bring own lunch.)
$35 per day 7:30 a.m. to 3:30 p.m.
$42 per day 7:30 a.m. to 6:00 p.m.
$145 per week 7:30 a.m. to 3:30 p.m.
$180 per week 7:30 a.m. to 6:00 p.m.
Any changes to camp attendance dates (including transferring days or absences) must be given with two weeks’ prior notice. We are not able to give refunds or cancel fees for missed days of camp with less than two weeks’ notice. If your child is unable to attend camp due to medical reasons, you may receive a full refund by providing a doctor’s note. Days may be added with one week’s notice. Drop-ins will not be accepted.

For staffing and planning purposes, please check the weeks your child will be attending. If your child is not attending the full week, please list the days they will be attending.
I understand that when participating in Christian School District Summer Camp activities the registrant may be photographed for print, video or electronic imaging. I understand that the images may be used in promotional and fundraising materials, news releases and other published formats, and will be the sole property of Christian School District.
By submitting this form, I consent for any of my children listed below to participate in any activity or trip sponsored by Christian School District Summer Camp. In case of medical need, I authorize Christian School District Summer Camp to arrange for medical or dental services for me and any of my children listed below. I agree that any such expense will be my obligation.
By submitting this form, I, individually, and in my capacities as parent or guardian waive, release, and indemnify Christian School District Summer Camp and its agents, directors, employees, and volunteers (collectively, the “Released Parties”) from all claims or liability which have arisen or may arise from any Christian School District Summer Camp activity or trip and which involves any damage, loss, or injury to me, my spouse, any of my children, my property, or the property of any of my children. In the same capacities, I promise not to sue any of the Released Parties for any such claims or liability. This waiver, release, indemnification, and promise not to sue, does not apply to claims of criminal conduct or gross negligence.
This Activity Release is revocable prospectively only in writing and signed by me which bears the date that the revocation is delivered to Christian School District.
